Here are some of the most commonly used forms. You can either download and complete these forms as required and return to school. Alternatively, paper copies of all forms and documents are available from the School Office.
Absence Request Form
Please use this form if you need to request a special leave of absence for your child.
Please note that you are committing an offence if your child doesn't attend school regularly and the school hasn't agreed the absence. Only exceptional circumstances where absence cannot be avoided will be authorised. If your form is not authorised, your child will be noted as having an Unauthorised Absence on the register.
If the school is unable to authorise the absence and the child is still taken out of school, you may receive a £60* Penalty Notice per parent for each child. *(£60 if paid within 21 days, increasing to £120 if paid after 21days and before 28 days)
This form should not be used for illness, medical or dental appointments. Pupils should never miss school for reasons like shopping, birthdays or to wait in for a repair person or a delivery.
You will receive an email to let you know if your request has been authorised.
Early Years and Infant Pupil Premium Checker
This form is only for parent and carer's of children in Nursery, Reception, Year 1 and Year 2 classes to check their child's eligibility for Pupil Premium funding. For pupils in Year 3 and above, please visit Bristol City Council's Free School Meals webpage.

Alliance in Partnership (AiP) Allergen Menu Request
If your child has a specific dietary need or allergy, our school meal provider, Alliance in Partnership (AiP), are able to supply specific menus tailored to their needs. Please complete the Allergen Menu Request Form below and return it directly to allergens@ainp.co.uk and also copy in the school office.
